Health Sciences Admission Requirements

Admission Criteria
  1. Complete university admission requirements to include submission of official transcripts from all colleges/universities attended.
  2. Have a minimum 2.50 cumulative GPA for all college coursework (including transfer work and excluding developmental courses) and be in good academic standing.
  3. Complete required prerequisite courses for the dental hygiene program with a minimum of C or higher in each course prior to the application deadline.
  4. Take the ATI-TEAS entrance exam (more information to come)
  5. Submit Dental Office Observation Verification Form. (Verification forms may be used for two years after the dates of observation.)
  6. Students may not attempt any course more than twice and be eligible for admission into the dental hygiene program.
Application
Admission into the dental hygiene program is competitive. Students who meet the minimum criteria identified above are eligible to submit an application for admission.   
  1. Application deadline is May 20 for fall entry into the program
  2. Eligible students must submit an application for admission to the advising coordinator for the College of Health, Education, and Human Sciences prior to the application deadline. 
Selection
Selection is based on the following weighted system:
 
Phase One
  • 40 percent - Quality points earned for prerequisites on the dental hygiene degree plan (College credit through CLEP or AP for a course will not be used for ranking purposes.)
  • 30 percent - Score earned on the ATI-TEAS Exam  
  • 30 percent - Cumulative GPA   

In the event that applicants have identical ranking scores, priority will be given to the student with the highest cumulative GPA. The top 25 applicants after Phase One of the selection process will proceed to Phase Two.

Phase Two

  • Interview - Interviews will be conducted by a dental hygiene selection committee. Candidates will be evaluated based on overall professionalism, oral and written communication skills, and the ability to meet the program requirements. 
  • The panel will select a maximum of 16 students who demonstrate the potential to:
    • succeed in the program;
    • meet the didactic and clinical rigors of the program;
    • demonstrate the necessary interpersonal skills to work as a team and interact with clients of varying ages and backgrounds;
    • successfully complete the National Board and clinical licensure examinations; and
    • be a viable part of the healthcare and dental fields.

To be considered for admission to the dental hygiene completion program, students must meet the following criteria:
  1. Complete university admission requirements to include submission of official transcripts from all colleges/universities attended.
  2. Submit official transcript showing successful completion of an associate degree in dental hygiene from a CODA-accredited program.
  3. Have a minimum 2.50 cumulative GPA for all college coursework (including transfer work and excluding developmental courses) and be in good academic standing.
  4. Complete required prerequisite courses for the program with a minimum grade of C or higher in each course prior to fall or spring entry.
  5. Have a current, unencumbered dental hygiene license with a local anesthesia permit in their state of residence that must be maintained throughout the completion of the A.A.S.-B.S.D.H. program.

Admission Criteria
1. Complete university admission requirements to include submission of official transcripts from all colleges /universities attended
2. Have a minimum 2.50 cumulative GPA for all college coursework (including transfer work and excluding developmental courses) and be in good academic standing.
3. Complete required prerequisite courses for the program with a minimum grade of C or higher in each course prior to fall entry.
4. Submit an official transcript showing successful completion of a Joint Review committee on Education in Radiologic Technology (JRCERT) approved associate degree program in radiologic technology
OR
Receive credit by articulation. Registered radiologic technologists who do not possess an associate degree may receive 40 imaging sciences credit hours by providing documentation of:
  • Graduation from a JRCERT-approved school of radiologic technology;
  • A passing score on the American Registry of Radiologic Technologists certification examination; AND
  • Successful academic performance (C or better) on 15 semester credit hours in the B.S.I.S. general curriculum.
5. Hold a current valid, unencumbered ARRT certification.

Selection
A maximum of eight (8) students will be admitted each fall. Selection is based on the following weighted system:
  1. Cumulative GPA for all college-level work
  2. Personal interview
  3. Selection preference given to those who are near successful completion of the general education curriculum and the imaging sciences core courses
NOTE: In the event that applicants have identical ranking scores, priority will be given to the student with the highest cumulative GPA. The top applicants will be offered positions for the available seats in the DMS program. Meeting the minimum criteria for admission or taking courses on the DMS degree plan does not guarantee admission into the program. Students not admitted must reapply to be considered for another semester. Students identified as alternates are not guaranteed a position in the next class.
 
Residency Requirements
Thirty (30) of the last 36 credit hours of the B.S.I.S. program must be completed at UAFS.

Admission Criteria
To be considered for admission to the BSN program, students must meet the following criteria:
  • Complete university admission requirements to include submission of official transcripts from ALL colleges/universities attended.
  • Have a minimum 2.50 cumulative GPA for all college coursework (including transfer work and excluding developmental courses) and be in good academic standing.
  • Complete required prerequisite courses for the program with a minimum grade of C or higher in each course prior to fall or spring entry. Prerequisite courses are the first four semesters as listed on the degree plan for a total of 62 hours.
  • Students may not attempt any course more than twice and be eligible for admission into the nursing program.
NOTE: RN licensure requires a state and federal criminal background check. Students who have been convicted of a crime, including those that are sealed or expunged, must seek permission from the Ï㽶ÊÓƵAPP State Board of Nursing to write the licensure exam to become a registered nurse. 2. Meet minimum skill levels in math, English, and reading according to the following criteria:
  • Coursework indicating completion of MATH 1403 or higher
  • Coursework indicating completion of ENGL 1203
  • ACT or COMPASS reading scores (taken in the last four years) or coursework indicating no reading required
3. Have a minimum 2.50 cumulative GPA for all college coursework (including transfer work and excluding developmental courses) and be in good academic standing.
4. Complete required prerequisite courses for the radiography program with a minimum of C or higher in each course prior to fall entry.
5. Take the ATI-TEAS entrance exam (more information to come)
 
NOTE: The application process for licensure requires a state and federal criminal background checks. Students who have been convicted of a crime, including those that are sealed or expunged, must seek permission from the American Registry of Radiologic Technologists to write the certification exam to become a registered radiographer.
 
Selection

NOTE: In the event that applications have identical ranking scores, priority will be given to the student with the highest cumulative GPA. The top 30 applicants after Phase One of the selection process will proceed to Phase Two.
Phase Two
  • Interview
  • Student Health Statement/Medical Release Form
NOTE: Following the completion of Phase Two, the top applicants will be offered positions for the available seats in the radiography program. Meeting the minimum criteria for admission or taking courses on the radiography degree plan does not guarantee admission into the program. Students not admitted must reapply to be considered for another semester. Students chosen as alternates are not guaranteed a position in the next class.

To be considered for admission to the PN program, students must meet the following criteria:
  1. Complete university admission requirements to include submission of official transcripts from ALL college/universities attended.
  2. Meet minimum skill levels in English, reading, and math according to one of the following criteria: (Scores must be available by application deadline.)
    • ACT, Aspire, COMPASS, SAT, or ACCUPLACER English scores (taken within the last two years) or course work indicating placement into ENGL 0263 Transition to College Writing and Reading or higher
    • ACT, Aspire, COMPASS, SAT, or ACCUPLACER reading scores (taken within the last two years) or course work indicating placement into ENGL 0263 Transition to College Writing and Reading or higher
    • ACT, Aspire, COMPASS, SAT, or ACCUPLACER math scores (taken within the last two years) or course work indicating placement into MATH 0304 Beginning Intermediate Algebra or higher
  3. Have a minimum 2.00 cumulative GPA for all high school and college course work (including transfer work and excluding developmental courses) and be in good academic standing
  4. Students may not attempt any course more than twice and be eligible for admission into the nursing program
  5. Complete the ATI-TEAS entrance exam (more information to come) with a minimum score of 200. (Applicants may only take this exam once per year.)
NOTE: PN licensure requires a state and federal criminal background check. Students who have been convicted of a crime, including those that are sealed or expunged, must seek permission from the Ï㽶ÊÓƵAPP State Board of Nursing (ASBN) to write the licensure exam to become a licensed practical nurse. Selection

  • Score earned on the ATI-TEAS 
NOTE: In the event that applicants have identical ranking scores, priority will be given to the student with the highest cumulative grade point average. Top applicants will be offered positions for the available seats in the PN program. Meeting the minimum criteria for admission or taking courses on the PN degree plan does not guarantee admission into the program. Students not admitted must reapply to be considered for another semester. Students chosen as alternates are not guaranteed a position in the next class.

The results of this ranking test (ranking against the other students who apply) will be used in combination with their student scores on the standardized college admission tests (Aspire, ACT, ACCUPLACER, or Compass) in order to select a maximum of 16 students and two alternates (in case a student changes their mind after being selected).

To be considered for admission to the program, students must meet the following criteria:

  • Complete university admission requirements to include submission of official transcripts from all colleges/universities attended
  • Have a minimum 2.00 GPA average for all college course work (including transfer work and excluding developmental courses) and be in good academic standing
  • Complete required prerequisite courses for the program with a minimum grade of C or higher in each course prior to fall entry; prerequisite courses are the first two semesters as listed on the degree plan for a total of 28 hours
  • Take the ATI-TEAS entrance exam

Admission Requirements
  • Meet all university admission requirements
  • Have a high school diploma or GED
  • Be at least 18 years of age at time of National Registry/State testing
  • Hold current American Heart Association (BLS for the Healthcare Provider) or American Red Cross (BLS for the Professional Rescuer) CPR certification

Additional Documentation

Students admitted into any health sciences program must submit the following additional documentation prior to entry:
  • Healthcare Provider Statement/Medical Release Form
  • Proof of immunizations
  • CPR certification: American Heart Association Basic Life Support for Healthcare Providers (BLS)
  • Criminal background check
  • Drug screen
Student Health Statement/Medical Release Form

Students accepted into any of the health sciences programs will be required to provide documentation of good physical and mental health by completing the Student Health Statement/Medical Release Form. Documentation of all health problems and a medical release will be required. Copies of immunization records and a valid CPR certification must be submitted. No student will be allowed to attend a clinical rotation without the required health data records.


Background Checks and Drug Screens
The application process for all health sciences programs and many clinical facilities requires a criminal background check and/or drug screen on students. Facilities have the authority to deny clinical training in their facility secondary to outcome of either screening. Students unable to practice in any clinical agency because of an adverse or negative background check and/or drug screen will be unable to complete the program objectives, thereby revoking admission into a program or halting continued progression in the student's program of study.
